РЕВМАТОИДНЫЙ АРТРИТ И СЕРДЕЧНО-СОСУДИСТЫЙ РИСК: РОЛЬ СИСТЕМНОГО ВОСПАЛЕНИЯ

евматоидный артрит (РА) представляет собой хроническое системное аутоиммунное заболевание, при котором воспалительный процесс выходит за пределы суставов и оказывает значимое влияние на различные органы и системы. В последние десятилетия накоплены убедительные доказательства того, что пациенты с РА характеризуются существенно повышенным сердечно-сосудистым риском. Ведущим патогенетическим механизмом, связывающим РА и сердечно-сосудистые заболевания, рассматривается хроническое системное воспаление, приводящее к эндотелиальной дисфункции, ускоренному атерогенезу и дестабилизации атеросклеротических бляшек. В статье рассматриваются современные представления о взаимосвязи воспаления, иммунных нарушений и сердечно-сосудистой патологии при РА, обсуждаются ключевые молекулярные механизмы, а также клинические и терапевтические аспекты данной проблемы. Особое внимание уделено роли провоспалительных цитокинов, иммунных клеток и метаболических изменений в формировании кардиоваскулярных осложнений.
